NDIS Quality and Safeguards Commission
For a concern about an NDIS provider or worker. You can complain anonymously, and you don't have to come to us first.
Positive or critical, we want to hear it — and we'd love to hear your success story too. Your name and contact details are completely optional: feedback is handled with care and confidentiality, and it can be 100% anonymous.
Clear, specific feedback helps us improve our services, and we aim to respond as promptly as we can. You're never obliged to accept a support — or a staff match — that doesn't meet your expectations; if something isn't right, tell us and we'll work on it with you.
Rather talk to someone? Call 03 4410 8243 or email reception@u-choose.com.au.
You can always go straight to an independent body, or bring someone to support you. Need an interpreter? Call TIS National on 131 450 and ask for us.
